一、项目内容：
### 计划采购一批急救和生命支持类设备。

* 以上每台设备的调研资料独立编制，即一台设备一套资料。
二、供应商资格条件：
#.符合《中华人民共和国政府采购法》第二十二条规定的条件；
#.在中华人民共和国境内注册的独立法人或其它组织；
#.具备医疗器械经营资格（要求证照在有效期内）；
#.所报名设备具备有效的医疗器械注册证或备案凭证，质量符合国家相关要求；
#. ### 报名设备的供货及售后服务能力；
#.三年内，企业在经营活动中没有重大违法记录、未发生安全责任事故。
三、调研文件内容：
各单位报名资料必须独立编制，调研文件，资料包括但不限于以下内容：
#.《法定代表人授权委托书》、《诚信承诺书》、《报价表》、《详细参数》、《详细配置》（已提供模板）；
#.其他资料：
（#）设备注册证、生产许可证、产品制造商营业执照，若所投产品制造商为中小企业时需提交中小企业声明函（格式见附件#）。
（#）产品彩页介绍（必须是彩色原件）、使用说明书等资料。
（#）相关产业发展、市场供给情况， ### 维护、升级更新、备品备件、耗材等后续方案，以及其他相关情况，后续供应的价格，以及后续采购的可替代性、相关产品和估价等内容。
（#） ### 执行的相关国家标准、行业标准、地方标准等标准、规范。
（#）设备使用过程是否需要一次性耗材， ### 保收费项目计算耗材占比；耗材是否可在招采子系统平台采购，是否开放（专机专用），是否能单独收费，社保报销比例等。
（#）使用及保养有无易损件，易损件的更换周期、价格，提供说明书。
（#）能否提供设备系统软件免费升级服务，开放设备相关数据接口， ### 权限等。
（#）若设备有选配功能或模块，可将设备基本配置及选配功能分别报价， ### 描述。
（#）设备用户名单。
（##）熟悉该设备性能的业务联系人及电话（ ### 方或代理商电话）。
（##）附件#.《问卷调查表》。
四、付款方式：
银行转账；签订合同签订生效，采购人收到中标人发货通知后，支付合同总金额的##%，整体验收通过，安装调试完成后，提供全额发票，支付合同总金额的##%（ ### 全保保修承诺函后）； ### 有技术培训服务后（需要采购人认可），质保期满后，支付合同总金额的##%。
五、验收要求：
#.验收与使用： ### 地、配置等条件下，中标人/ ### 家要保证设备、系统调试至采购人在实际工作中正常使用。 ### 后，才可验收进入质保期。如果中标货物属于《计量法》规定的强检计量器具的或属于压力容器的， ### 门出具的检定合格证书，检定费用由中标人负责。
#. ### 标准：
（#）符合中华人民共和国国家和履约地相关安全质量标准、行业技术规范标准、环保节能标准；
（#） ### 文件和响应承诺中采购人认可的合理最佳配置、参数规格及各项要求。
（#）货物来源国官方颁布标准。上述各类 ### 标准版本，中标人应向采购人提供详细的验收标准、验收手册； ### ### 精度校核。
六、参与须知：
